NDI ≥ 30% with imaging-confirmed structural cause (disc herniation with radiculopathy, myelopathy, instability) plus failure of conservative care is a typical surgical-eligibility threshold. Myelopathic features (gait disturbance, hand clumsiness, hyperreflexia) lower the threshold considerably.
