Why China for men's health?
Three reasons: (1) Volume — China's flagship urology departments operate at unmatched scale, with surgeon case volumes exceeding most Western private practices; (2) Cost — BPH surgery (HoLEP, TURP), prostate cancer surgery and andrology workup run 60–70% below US private pricing; (3) Equipment — robotic urology (da Vinci), HoLEP holmium laser, PSMA PET-CT staging, and microsurgical operating microscopes for vasectomy reversal and varicocelectomy are widely available.
What does BPH treatment look like?
Symptom-based stepped approach: lifestyle modification (fluid timing, caffeine reduction, bladder training); alpha-blockers (tamsulosin, silodosin, alfuzosin) ± 5-alpha reductase inhibitor (finasteride, dutasteride) for prostate > 30g; combination if symptoms uncontrolled; PDE5i (tadalafil 5mg daily) if concomitant ED; surgery if medical failure or complications. Surgical options: HoLEP (best for large prostate, gold standard), TURP (standard for medium prostate), Rezum (water vapor, less invasive), photoselective vaporization, simple prostatectomy for very large prostate. UroLift availability in mainland China is limited.
What about TRT (testosterone replacement therapy)?
Standard pathway: confirm true testosterone deficiency with 2x AM total testosterone < 300 ng/dL (10.4 nmol/L) plus symptoms; rule out reversible causes (obesity, OSA, opioid use, iron overload); discuss fertility implications (TRT suppresses spermatogenesis); confirm prostate health (PSA, DRE) and hematocrit; initiate TRT (testosterone enanthate / cypionate IM, transdermal gel, or testosterone undecanoate long-acting); monitor at 3, 6, 12 months. NMPA-listed TRT options include testosterone undecanoate oral and IM, testosterone enanthate, and gel formulations at substantially lower cost than US.
What about erectile dysfunction?
Standard pathway: history, comorbidity assessment (cardiovascular, diabetes, depression), medication review, testosterone level if low libido or non-responsive; PDE5i first-line (sildenafil, tadalafil, vardenafil — generic NMPA-listed at substantially lower cost than US); intracavernous injection therapy if PDE5i fails; intraurethral alprostadil; vacuum erection device; penile prosthesis (semi-rigid or 3-piece inflatable, AMS / Coloplast) for refractory cases. PDE5i + TRT combination considered carefully in patients with documented testosterone deficiency.
What about male infertility?
Workup: detailed history; 2x semen analysis with strict morphology; hormonal panel (testosterone, FSH, LH, prolactin, TSH, estradiol, SHBG); genetic workup (Y-chromosome microdeletion, karyotype, CFTR if azoospermia); scrotal ultrasound for varicocele or testicular pathology. Treatment: varicocele microsurgical repair if clinically significant; medical optimization (clomiphene, hCG); microsurgical TESE for non-obstructive azoospermia; ICSI for assisted reproduction. Note: IVF in mainland China is restricted to married heterosexual couples — see [IVF page](/ivf-china).
What about prostate cancer screening?
Standard recommendation: shared-decision PSA screening starting age 50 (45 for high-risk: African ancestry, BRCA mutation, family history). High-quality screening combines PSA + DRE; if abnormal, multiparametric MRI prostate before biopsy reduces overdiagnosis of indolent cancer.
