What does the evidence actually show for TCM and insomnia?
Best-evidence formulas: (1) Suanzaoren tang (酸枣仁汤, Ziziphus jujuba decoction) — multiple Chinese RCTs and a 2020 Cochrane-style systematic review (Birling et al., Phytomedicine 2020) suggest modest improvement in subjective sleep quality vs placebo, comparable to low-dose Z-drugs in short trials, with fewer next-day cognitive effects. Quality of evidence is moderate-low — most trials are small, single-centre, and short-duration. (2) Zaoren Anshen capsules (枣仁安神胶囊) — patent formula, NMPA-registered, mixed-quality RCT data. (3) Tianwang Buxin Dan (天王补心丹) — classical formula for ‘yin deficiency’ pattern, weaker but suggestive evidence. We do not claim TCM as primary therapy where moderate-to-severe insomnia, sleep apnoea, or major depression / anxiety is present.
Why come to China for TCM rather than buy formulas online?
Three reasons: (1) Class A teaching-hospital TCM pharmacies dispense formulas manufactured to NMPA-GMP standard with batch-level quality control — back-channel online sources frequently fail heavy-metal, pesticide and adulterant testing (FDA and EU import alerts); (2) classical TCM diagnosis (pattern differentiation, 辨证论治) requires in-person assessment including pulse and tongue diagnosis, which is integral to formula selection per the system; (3) hospital TCM departments coordinate herb-drug interaction screening with Western pharmacy — critical if you are on antidepressants, anticoagulants, anticonvulsants, statins or DM medications.
How is the workup done?
Standard 1–2 day pathway at a Class A integrative or TCM hospital: (1) Western sleep medicine assessment first — sleep history, ESS / PSQI / ISI scoring, screen for OSA (STOP-BANG), restless legs, parasomnias, depression/anxiety; if OSA suspected we order a home sleep test or refer to in-lab PSG before TCM. (2) TCM consultation — pattern differentiation by senior practitioner, typically 30–45 minutes including pulse and tongue diagnosis. (3) Formula prescribed — either decoction (汤剂, raw herbs prepared 1–2× daily) for highest individualization, granules (颗粒剂) for travel convenience, or patent formula (中成药) for simplicity. (4) Herb-drug interaction screen with Western pharmacy if you are on prescription medication. (5) 4-week follow-up plan — remote check-in or repeat visit.
What does it cost?
Indicative all-in: Western + TCM dual workup + first 2-week formula + bilingual report — $250–500. Decoction (raw herbs, daily) ~$3–8/day. Granules ~$2–6/day. Patent formula (中成药) e.g., Zaoren Anshen capsules ~$15–30/month. What about acupuncture for insomnia?
Acupuncture for chronic insomnia has been evaluated in multiple meta-analyses (most recently Yin et al., Sleep Medicine 2017; Zhao et al., Sleep Medicine 2020). Combined evidence suggests modest improvement in sleep quality vs sham acupuncture, with effect sizes generally smaller than CBT-I but comparable to low-dose hypnotics for some endpoints. Quality of evidence is moderate. We position acupuncture as second-line, integrated with sleep hygiene and CBT-I principles, not as a substitute for evaluating and treating sleep apnoea or major mood disorder.
What is NOT in scope?
We do not provide TCM-only management for: untreated obstructive sleep apnoea (we screen and refer); insomnia in the context of moderate-to-severe major depression, bipolar disorder, or active substance use disorder (these require psychiatric care); insomnia in pregnancy without obstetrics consultation; herbal management of serious chronic disease (e.g., chronic hepatitis or active autoimmune disease) where we recommend Western primary therapy and discuss herbs only as supportive (and only after interaction screen). We will tell you clearly when TCM is not appropriate.
Are formulas safe to bring home?
Yes, generally — finished patent formulas (中成药) in original NMPA-registered packaging and granule packets travel without issue in most jurisdictions for personal use. Raw herb decoctions are bulkier and harder to transport long-distance. We provide a bilingual prescription, English ingredient list with botanical names, and a physician’s letter for customs. A small number of TCM ingredients (e.g., ephedra / ma huang) are restricted in some Western countries — we do not prescribe these for patients returning to such jurisdictions.
